Early years support in the home for children with SEND

Early Support is a key working service available to families caring for children (aged 0-5) with a complex disability requiring them to be in contact with a wide range of health and social care professionals. It provides help with coordinating the care package for your child, including managing appointments and support accessing mainstream services. The family will have a chance to contact their key Worker whenever they feel they need support and the key worker will regularly update the family regarding work other professionals do.

Families will normally be referred into the Early Support Service by a professional (e.g. paediatrician). If eligible, an action plan will be drawn up outlining the support the family requires and how it will be provided.

Parents or carers of very young children with additional needs or recognised disabilities may benefit from the Portage Home Learning Service. Portage works in partnership with the child's family to build on the child's existing abilities and to foster the child's development. Portage Home Visitors visit on a weekly basis, and goals are discussed and set. The approach uses small steps to ensure success.
