Pathways to harm
The Department of Education have published a report analysing serious case reviews (SCRs) with findings for professionals working with children and families.
This study is the fifth consecutive analysis of serious case reviews in England undertaken by the same research team dating back to reviews from 2003-2005. The study considers a total of 293 SCRs relating to incidents which occurred in the period 1 April 2011- 31 March 2014. These most recent reviews are also analysed in the context of learning from SCRs over the ten years since 2003-2005. The aim of the study is to provide evidence of key issues and challenges for agencies working singly and together in these cases.
