About the service
We work with families to help them access support services for children with disabilities.
This includes:
- providing emotional support
- making referrals to other professionals
- helping with benefit and other applications
- liaising with other professionals
- co-ordinating services
Eligibility
A child under the age of six is eligible if they show a delay in more than one area of development, which could significantly affect their everyday living. This could include:
- global developmental delay
- a learning disability
- a sensory impairment
- a physical impairment
- chronic ill health or medical problems
- a serious life limiting or terminal illness
- a severe communication or language disorder
- an autistic spectrum disorder with associated learning disability
Referrals
Referrals can be made:
- directly by the family by phone or by email
- by professionals involved with the family by completing a referral form - these are available from the Key Worker Service based at St Quintin Centre
Assessment
We will contact you to make an appointment for an assessment.
