Community Learning Disability Team (CLDT) for People with Learning Disabilities

About the service

Community Learning Disability Team (CLDT) for people with learning disabilities works with people and carers to promote independence. They offer advice and support to people on every aspect of living with a learning disability including:

  • making choices, finding employment and being more independent
  • healthy eating advice, health action plans and looking after yourself
  • getting the most out of local health and social services
  • budgeting and direct payments
  • meeting other people and taking part in activities
  • learning new skills
  • short breaks for you and your carer

Our learning disability services are run in people's homes, health centres, nursing and residential homes throughout Barnet, Hammersmith & Fulham, Kensington and Chelsea, and Westminster.

How to access the service

This service is for people who have a learning disability, are 18 years or older or moving from children to adult services and have a GP in Kensington and Chelsea.

If you have a health related concern regarding your learning disability, you can refer yourself or ask someone refer you to the service. They can be your family, friend, GP, social worker or other professional.
