Enquire about housing assistance

Complete the online housing assistance form if you are homeless, or you believe you are threatened with homelessness. This form can also be completed if you require housing assistance, for example:

  • if you are unsafe in your home and help to move out
  • if you want to move into private rented accommodation
  • if you want to join the Sheltered Housing Register or the Council Housing Register.

Form guidance

The form can be completed by yourself, or family or friend, or a professional who is supporting you. Once the form has been completed it will be sent to our Housing Solutions Team who will determine what assistance (if any) can be given. 

Please complete the form with as much information as possible, as this will help us to understand your housing situation. You must answer all mandatory questions, they are the questions marked with a star symbol (*). If you do not complete these questions, you will not be able to move onto the next section. 

Do not complete this form if your situation is an emergency and you require an urgent response. For example, if you are:  

  • you are at risk of violence 
  • you have nowhere to sleep tonight
  • you need to leave your temporary accommodation 
  • you are are a victim of a flood or fire

Please note the following before you complete the form: 

  • You cannot save the form and return to it at a later stage, it must be completed in full. 
  • The form asks you for personal information, including an email address, telephone number and national insurance number, address history, benefit and employment information. Please have these details to hand. 
  • You have the option of uploading documents at the end of the form – we would strongly encourage you to upload as much supporting information as possible, but this is not mandatory.
